To be eligible, facilities must employ at least fifty employees, have at least 70% of their staff voluntarily self-identify as Black, Hispanic, or Asian, and maintain an average annual occupancy rate of at least 80%. Facilities will receive increased capitated rates of payment, rising from 31% to 50% over five years. The commissioner will submit an annual report evaluating the program's fiscal impact and outcomes.
