New Jersey A4397 allows a driver to revert their name to their birth name using a death certificate.

New Jersey A4397 requires the New Jersey Motor Vehicle Commission to accept a death certificate as proof of a name change for a driver whose spouse has died. The driver must also submit their most recent marriage certificate and attest that they have not been convicted of a crime, have no pending criminal charges, and are not changing their name to avoid or obstruct criminal prosecution, avoid creditors, or perpetrate fraud.
