New Hampshire SB408 amends insurance policies to require coverage for prosthetic devices, including activity-specific ones, for individuals over 19 years of age who reside in the state. Insurers may limit coverage for activity-specific prosthetic devices to one per five years and may apply utilization management. Medically necessary prosthetic devices are not subject to annual limits. Coverage includes all necessary materials, instruction, and repair or replacement of the device. This act does not apply to plans available through the Small Business Health Options Program (SHOP).
