Works to prescribe and furnish the employment certificate form through the department's website or local field offices. The employment certificate form must include a signature line for the minor, a statement attesting to parental consent, and lines for the parent or legal guardian to sign and date the certificate. The certificate is valid only for the employer listed on the form.
