Kentucky SB216 amends voter registration forms to include additional personal information.

Kentucky SB216 modifies the voter registration process by specifying that all forms must be prescribed and furnished by the State Board of Elections. The registration form now includes the voter's name, date of birth, political party membership or independent status, Social Security number, and mailing and residence addresses. Importantly, no voter will be denied registration for not providing a Social Security number. Additionally, any form requiring a voter's signature must allow for verification by the signer.
